Terms of Agreement for Month-to-Month TRT (Testosterone Replacement Therapy)

1. Agreement Overview:

This Terms of Agreement ("Agreement") governs the provision of TRT services provided on a month-to-month basis. By enrolling in and utilizing these services, you agree to be bound by these terms, which include payment obligations and cancellation policies as detailed herein.

2. Services Provided:

You will receive Testosterone Replacement Therapy services tailored to your individual health needs as assessed by our medical professionals. These services may include, but are not limited to, consultations, hormone evaluations, prescriptions, and ongoing monitoring and adjustments.

3. Payment Terms Monthly Billing: Payment for TRT services is due monthly. You will be automatically billed each month on the date of your initial enrollment. Payment Methods: We accept payments via credit card or debit card. Please ensure your payment information is kept current to avoid interruptions in service.

4. Modifications to Terms:

We reserve the right to modify these terms and conditions at any time. Any modifications will be effective immediately upon posting the revised agreement on our website. Your continued use of TRT services after any such changes shall constitute your consent to such changes.

Cancellation and Payment Policy for Month-to-Month TRT Agreement:

Monthly Payments: Your month-to-month TRT (Testosterone Replacement Therapy) agreement involves recurring monthly payments. You will be billed on a monthly basis for the duration of your agreement.

30-Day Notice for Cancellation: If you wish to cancel your month-to-month TRT agreement, you must provide a minimum of 30 days' written notice prior to your next billing date. This notice must be submitted in writing to ensure the cancellation is processed correctly.

Effect of Cancellation: If you cancel your agreement with less than 30 days' notice before your next billing date, you will be responsible for the payment for the next month. This charge is necessary due to the scheduling and resource allocation requirements associated with your treatment plan.

Refunds: There are no refunds for partial months or for months where the service has been utilized, regardless of the number of days used. Please plan accordingly when deciding to cancel your services.

Disputes: Any disputes with the credit card company used for payment after the procedure is completed, patients acknowledge that they will waive their right to portions of the HIPAA policies and that portion of their medical records will be used to clarify the dispute with the credit card company. This concludes all documents that authorize the procedure by the patient.

You hereby agree to hold MD Hair Labs & Vitality, its doctors and affiliates, harmless from any hacking or any other unauthorized use of your personal information by outside parties.
